If signing the Subscription Agreement to hold the membership interest as Community Property (Tenants in Common or other form of joint ownership), please complete the above form. For community property states, or in cases where the spouses/partners hold property as a community, we require signatures from both individuals.

Upon receipt of your submission, we will process the request and send the form via Adobe Sign for electronic signature.

Both individuals will receive an email for signature in sequential order. If you have questions about how you and your spouse/partner hold title to property, please seek guidance from competent legal counsel.
